Incident Reporting Guide

A workplace is encouraged to use the internal responsibility system which includes the employer, the supervisor, and the workers to develop and maintain a safe workplace. This includes Joint Health and Safety Committees.

‍EIRs are Employee Incident Reports These are the only official documentation of an incident which belongs to an EA. Each incident is important as these records are the basis for WSIB claims and the Supervisor is responsible for addressing whatever caused the illness or injury to occur. EAs must be given work time to complete these forms and debrief with the Supervisor. An Employee Incident documents physical injury and/or mental harm. Exposure over time can be documented as well as incidents which are a ‘near miss’.

To access the Incident Report in eBase, go to MyHDSB.ca,  choose “Applications”, then “eBase works”. 

How to Send/Share EIRs from the Ebase System

  1. On the FORM LOGIC page of ebase
  2. Click on the EIR you wish to print
  3. In the top right corner, click on ACTIONS
  4. Click PRINT PDF
  5. The PDF will open in a new window.
  6. Top right corner, you can download, to send to the union office, and print.

You may have to click on STATUS from the menu on the left on the FORM LOGIC page to find older or "COMPLETED" EIRs.
